Licensed Practical Nurse (LPN)

Under the direction of the RN, the Licensed Practical Nurse (LPN) delivers high-quality, patient-centered nursing care within a collaborative Primary Care Team. The LPN supports health promotion, disease management, and care coordination while advancing Fair Haven Community Health Care’s mission of providing accessible, culturally competent healthcare.

Key Responsibilities

  • Provide direct patient care in clinic and via telephone, including assessments, medication administration, vaccinations, and patient education.
  • Collaborate with RNs, providers, Medical Assistants, and interdisciplinary team members to develop and implement individualized care plans.
  • Coordinate patient care, specialty referrals, anticoagulation management, and communication with community agencies and home health providers.
  • Perform medication reconciliation, review laboratory results with patients as directed, and conduct follow-up assessments.
  • Document accurately in the electronic medical record (EMR) and manage patient forms, messages, and care-related communications.
  • Delegate and provide clinical oversight to Medical Assistants, supporting workflow efficiency and quality patient care.
  • Respond to emergencies, maintain clinical supplies and medication inventory, and ensure compliance with safety and infection-control standards.
  • Participate in quality improvement initiatives, team huddles, departmental meetings, and patient-centered care activities.
Qualifications
  • Current Connecticut Licensed Practical Nurse (LPN) license required.
  • Graduate of an accredited nursing program.
  • Minimum 2 years of nursing experience required; primary care or fast-paced ambulatory care experience preferred.
  • Current BLS certification.
  • Strong clinical assessment, critical thinking, and problem-solving skills.
  • Excellent verbal, written, and electronic communication skills.
  • Ability to work independently and collaboratively in a team-based environment.
  • Proficient with EMR systems and computer applications.
  • Commitment to compassionate, culturally competent, patient-focused care.
  • Bilingual English/Spanish preferred.
