Position Summary
The Regional Case Manager, Latino Outreach will provide culturally enhanced case management services for individuals with substance use or co-occurring disorders. This position will focus on recovery-oriented services that increase individuals access to equitable care, connect individuals to support services, improve individuals’ quality of life, decrease stigmas and promote health equity with a focus on Latino populations/groups. An emphasis will be placed on targeting culturally and linguistically competent services with a focus on addressing identified disparities in the global systems of care for Latino populations. The Regional Case Manager will collaborate extensively with outside service providers, and make appropriate internal/external referrals.
Key Responsibilities
Reporting to the Director of Addiction Medicine and with close collaboration with the Department of Care Coordination and Integrated Support Programs, the Regional Case Manager of Latino Outreach carries the responsibility for providing case management and outreach services to clients throughout Region 2 of the CT Department of Mental Health & Addiction Services (DMHAS), with a focus on reaching the Latino population. The focus is on reaching and helping Latinos facing unhealthy substance use. This includes, but is not limited to the following:
