Job purpose

The Maternal Health Care Coordinator is a vital member of the midwifery and care coordination teams. This role works closely with the Certified Nurse Midwives (CNM), nurses and care coordinators to improve access to quality, patient-centered care, which leads to better health outcomes. The Maternal Health Care Coordinator also works to identify and mitigate social barriers that may affect a patient’s health and well-being.

Duties and responsibilities

Reporting to the Care Coordination Program Manager, the Maternal Health Care Coordinator will work collaboratively with the midwifery team to coordinate prenatal and postpartum care for FHCHC midwifery patients. The Maternal Health Care Coordinator will focus on achieving healthy outcomes through coordination of high quality of care, and connection to community resources.

Primary responsibilities include but are not limited to:

  • Facilitate access to prenatal care, including outreach to patients that have missed appointments, coordinate labs and imaging as needed.
  • Ensure appropriate scheduling of prenatal and postpartum visits, as well as prenatal groups.  Conduct reminder calls, screen for and mitigate barriers to attendance.
  • Participate in prenatal groups and provide patient and provider support as assigned by manager.
  • Complete forms required for clinical care and addressing social determinants of health (e.g. durable medical equipment, transportation, utility forms).
  • Respond to patient calls relevant to role as Care Coordinator and as assigned by manager.
  • Screen for social determinants of health with every prenatal patient not connected with the Healthy Start Program. 
  • Link patients with identified social needs to community resources.
  • Assist patients with insurance enrollment, or other patient assistance programs.
  • Attend relevant trainings as required and assigned.  Attend morning huddles, weekly and monthly team meetings.
  • Document client interactions in the EPIC electronic health record and communicate securely with other team members and clinicians.
  • May provide interpretation (if qualified to do so).
  • Maintain strict adherence to all deadlines including report deadlines and timely completeness of documentation. 

Qualifications

Associates Degree in a health related field is required, relevant experience may be considered in lieu of degree.  A Bachelor’s degree, and a certification in Medical Assisting preferred. Bilingual English and Spanish, strong knowledge of medical terminology and relevant procedures and a valid CT driver’s license and/or access to reliable transportation is required.    

Experience in Care Coordination; particularly in maternal health; using EPIC electronic health record is also preferred.

The ideal candidate will possess strong computer skills, including word processing and data entry, and demonstrate the ability to work independently.
