Job purpose

Reporting to the Program Manager for Cancer Care, the Cancer Care Community Health Worker is the community ambassador, educator, and navigator for cancer prevention at Fair Haven Community Health Care (FHCHC). The role coordinates FHCHC’s patient-facing efforts at preventing cancers of all types, including breast, cervical, colorectal, and lung. The role effectively communicates about cancer prevention in ways that resonate with communities served by FHCHC.

This is a grant funded position with expectation for continued support.

Duties and responsibilities

  • Perform direct outreach to patients requiring cancer screening to provide education and navigation
  • Work collaboratively with community partners to ensure access to screening for key population groups
  • Serve as a community-facing point person for cancer prevention in the communities served by and surrounding FHCHC
  • Review gaps-in-care reports generated by the electronic health record to identify target patients
  • Work collaboratively with clinical teams to meet cancer screening goals
  • Screen for and address the social determinants of health that may prevent access to cancer screening among FHCHC patients
  • Contribute to social media and other media campaigns for cancer screening education
  • Performs other duties as directed by supervisor

Qualifications

The successful applicant will have a bachelor’s degree (or associate’s degree with relevant work history) and demonstrated knowledge of communities in New Haven City. Bilingual English/Spanish is required. Proficiency with Microsoft office is also required. Experience in health care and experience with electronic medical records systems are highly desirable

 Physical requirements

Physical Demands: Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, pulling medical records, Mental Demands: Ability to cope with continual changing priorities under potentially stressful conditions. Manual Dexterity Required:  Ability to use a keyboard, telephone.
